Summary
Endoscopy is a complex, multidisciplinary medical and technical activity that requires a high degree of coordination between the various professions involved. Given the current impetus placed on safer care, a preliminary risk analysis was undertaken in a healthcare facility concerning the pathway followed by patients planned for an endoscopic hepatic/gastric/enterological examination under GA. The observations made in the units and the review of the undesirable events reported enabled us to define the process and the associated generic risks. We were also able to establish a map of hazardous situations and draw up a table of scenarios. These various elements were analysed and validated by a multidisciplinary workgroup. Criticality was evaluated for each scenario to identify high-priority risks and determine corrective measures. The implementation of 5 lines of improvement enabled us to control all the risks previously deemed unacceptable. Residual risk evaluation and a mean- and long-term follow-up of the implemented measures will need to be organised. This study enabled healthcare professionals to identify the risks inherent to their activity, using a multidisciplinary approach, in agreement with the regulatory request for improved care quality and safety.
